Our rehabilitation program assists residents with a variety of ailments including, but not limited to:

Our state-of-the-art rehabilitation program creates custom care plans for each resident based on their individual needs. Our team is dedicated to ensuring residents experience whole body improvement, increased strength, and overall improvement in quality of life throughout their rehabilitation program.

Secure memory Care Unit

We have structured, supervised “failure-free” activities, suitable and enjoyable for these residents that are offered at least 12 hours each day. There is also an inviting, safe, enclosed outdoor area for residents to enjoy.

Crescent Manor Rehabilitation Center offers the privacy and security of a "closed" unit that is certified as an ACE (Alzheimer's Care Excellence) facility by the State of Vermont. Our staff members have to complete a rigorous training program and participate in monthly training to ensure the continued expertise necessary to provide the highest level of care to patients with Dementia. We recognize that the environment plays an important role in helping the resident feel safe and maintain as much independence as possible. We have devoted an entire wing of Crescent Manor exclusively to the care of residents with Dementia and created a fully functional independent unit with its own special dining and activities rooms.

Huntington's Disease Special Care Unit

Our Huntington's Disease program offers residents the maximal freedom of movement and independent decision-making. A "Primary Care" Nurse with specialized training in the management of this disorder will provide "hands-on" care as well as coordinating the work of ancillary personnel including physical and occupational therapy, speech therapy, dietary, social services and activities staff. Deer Oaks Mental Health provides psychological assessment and support services.

Crescent Manor is the only long-term facility in Vermont licensed by the State to provide specialized care for individuals with Huntington’s Disease. The program is tailored to meet the individual needs of each resident and help preserve the highest possible level of physical and psycho-social function.

WOUND CARE

Crescent Manor Rehabilitation Center has a comprehensive Wound Care program. We approach wound care in a multidisciplinary style with an evidence-based clinical approach to ensure effective wound care management including advanced techniques and treatments. We also ensure ongoing staff education on the causes and prevention of wounds to ensure the health and well-being of all of our residents.
