Specialized Care
At Crescent Manor Care Centers, each staff member shares the same goal: to consistently provide the highest standards of treatment for every patient under our watch. We take our responsibilities very seriously, and are proud to offer the very best around-the-clock care for residents with a broad range of conditions and specialized requirements. Together with physicians and family members, our dedicated group of nurses, clinical aides, therapists and counselors works collaboratively to develop an individualized plan of care for each resident. We are flexible and responsive to the changing needs of our residents, and frequently review and adjust courses of treatment. Our staff is trained to give skilled nursing care, as defined and approved by Medicare. The scope of treatments range from intravenous fluid administration to pain control measures to wound treatment, among many others. Our rehabilitative programs are carried out by licensed and/or certified therapists, and may be covered by Medicare. Progress is monitored closely to ensure residents continue to achieve maximum results throughout recovery. Additional therapeutic activities such as music, games and entertainment promote socialization and encourage motivation to foster a positive attitude towards treatment and recovery. Our success speaks for itself: nearly a third of those receiving rehabilitative services are able to return home. Sub-Acute Care Unique in scope and capabilities, our sub-acute care program provides intensive, highly skilled treatment for individuals recovering from serious illness or injury. Many of our residents come straight to Crescent Manor from the hospital. Complex nursing therapies, including pain management techniques, feeding technologies and other rehabilitation initiatives, help each resident achieve the highest possible level of recovery and independence. Crescent Manor's commitment to this exacting course of treatment stands alone among facilities of our size, and shows our ongoing pledge to meet and surpass the needs and expectations of every patient who comes through our doors. Sub-acute care is covered by Medicare. Dedicated Dementia Care Our residents with dementia live on a private, fully secured wing devoted exclusively to their care. We are united in our efforts to support the self-esteem and dignity of every resident suffering the devastating effects of this disease. Certified as an Alzheimer's Care Excellence (ACE) facility by the State of Vermont, our program is staffed by individuals with special interest and skills in dementia. Each employee must not only successfully complete a rigorous training program and participate in monthly training sessions, but also demonstrate deep understanding and compassion for this special group of residents. Families depend on our social service staff for advice, assistance and counseling. Many also benefit from participating in our active family support group. We are proud of our commitment to be a restraint-free facility. Palliative Care Palliative care allows individuals with advanced incurable illness and limited life expectancy to choose a course of treatment that is directed at comfort rather than cure. Our palliative care philosophy embraces holistic concepts of care focused on body, mind and spirit. Our goal is to alleviate pain and relieve symptoms while enhancing overall comfort and peace. Treatment may include intravenous fluids, tube feedings, parenteral or topical administration of medications to control pain, and other measures to achieve a greater sense of well-being. Crescent Manor staff has special training in this holistic approach to terminal illness, and often works in conjunction with local Hospice services to provide the most dignified and serene course of care. A key focus of our program is the enhancement and encouragement of quality time together for families and residents at the end of life. We offer private rooms for quiet visits, meals on request and cots for overnight stays. Clergy and spiritual counseling services are readily available and always welcome. Short-term Respite Care As part of our commitment to quality service, Crescent Manor offers short-term respite care for individuals who are normally looked after full-time at home. Caregivers who need a break can depend on Crescent Manor's superior staff and facilities to take over all daily duties for as short or long a time as needed. We welcome any individual 18 years and older in need of 24-hour supervision and medical care. This includes those who require assistance with daily activities, specialized food preparation and medication monitoring, among other needs. We also offer complex care for patients with a range of degenerative conditions, such as multiple sclerosis, cerebral palsy, dementia, stroke and paralysis. |
